After having felt like they haven't blown enough of American taxpayer money, they decided yet again to take a risk and let it ride. After Challenger, Apollo 13, Columbia, and the probe sent to study the climate on Mars where they forgot to convert imperial units to metric, there would be no reason to question a NASA judgement call. So when they saw an issue with the foam on the space shuttle Discovery you would see no reason for them to call it back. Except for the fact that Columbia is believed to have been doomed for the exact same reason. So for your constant screw ups and great waste of my money, this award goes to you NASA.
